How to access my medical records

Therefore you are required to make written request on this form as well as attach your valid personal ID. If you do not feel comfortable attaching your ID you can collect your documentation in person and present it at the reception.


Please note that doctor needs to approve release of your documentation and as per guidelines might choose to withheld parts of you record.


Due to the nature of this procedure it can take up to 30 days to complete this process.
