How to Request Sick Leave Certificate and Prescription Renewal from Your Regular Doctor

If you need a sick leave certificate and prescription renewal from your regular doctor, but the doctor is temporarily unavailable, you will need to wait until the doctor returns and book a consultation.

When Your Doctor Is Temporarily Unavailable

Occasionally, your regular doctor may not be available at the clinic for a period of time. During this time:

• Sick leave certificates and prescription renewals cannot be issued without a consultation
• You will need to book an appointment when your doctor returns to the clinic
• Documentation cannot be prepared in advance without a scheduled consultation

How to Book a Consultation

To book a consultation when your doctor returns:

1. Call our reception team at 018686121
2. Let us know:
3. • Which doctor you would like to see
   • That you need a sick leave certificate and prescription renewal
   • Your preferred appointment date and time
3. Our team will check availability and book an appointment for you

Collection of Documents

Once your consultation is complete:

• Your sick leave certificate and prescription will be prepared
• You can collect documents from reception during clinic opening hours
• Your prescription may also be sent electronically to your preferred pharmacy

Important Notes

• A consultation is required to issue sick leave certificates and renew prescriptions
• Documents cannot be issued without an appointment, even if you are a regular patient of the doctor
• Please book your appointment as early as possible if you need documentation by a specific date
